On occasion, pets may require treatment for conditions that develop or worsen while boarding (such as ear infections, gastritis, urinary tract infection, etc.). While every attempt is made to contact you at the emergency number provided, at time it is difficult to accomplish this.

Please review and sign one of the options below and date below: *

I wish for my pet to begin treatment while boarding, after contact has been attempted, even if I am unreachable. Diagnostics and treatments will be directed at alleviating the current problem at this time (such as medication for diarrhea, ear infections, etc.). Treatment will not exceed what is medically reasonable for said condition.
I do not wish for my pet to begin treatment without my authorization. I must be contacted prior to initiation of treatment (except in the case of life-threatening emergency).
